[email protected] S long-term peritoneal dialysis (PD) requires preservation of the transport function of the peritoneal membrane. Adequate dialysis is not only a matter of achieving the targets for Kt/Vurea and creatinine clearance, but also—and maybe even more importantly—of maintaining an optimal volume status. The development of ultrafiltration failure (UFF) remains one of the most important causes of treatment drop-out. It is also associated with a poor prognosis. Evidence is mounting that the incidence of UFF increases with time spent on PD, suggesting that the peritoneal membrane is progressively damaged during PD. However, the nature and the causes of the structural and functional alterations of the peritoneal membrane in long-term PD remain to be fully defined. Undoubtedly, continuous exposure to unphysiologic dialysis solutions is an important pathogenetic element in this respect. In addition, the repeated acute inflammatory damage conferred by recurrent peritonitis episodes and the chronic low-grade inflammation associated with the uremic state are likely to contribute. The present article reviews the extant studies on the functional and structural alterations of the peritoneal membrane in long-term PD. In addition, it focuses on the evidence for dialysate bioincompatibility in the genesis of these changes. Finally, therapeutic strategies with the potential to preserve long-term peritoneal membrane function and structure are addressed.

Recent advances in the management of peritoneal dialysis patients
Peritoneal dialysis is a form of kidney dialysis that is used to remove accumulated metabolic waste products and water in patients with end stage kidney disease. Long-term exposure to high concentrations of glucose and its by-products, both found in peritoneal dialysis fluid, has been implicated in contributing to peritoneal damage over time, in turn limiting long-term use of the technique. New...
متن کاملA Comparative Study on Using Coiled Versus Straight Swan-Neck Tenckhoff Catheters in Patients Undergoing Peritoneal Dialysis
The key to successful long-term peritoneal dialysis (PD) is permanent and safe access to peritoneal cavity. The two most commonly used Tenckoff catheters for PD are the straight and coiled catheters. The present study was undertaken to assess the catheter survival, catheter associated infections, and all cause mortality and to compare the straight with coiled catheters in PD. During April 1997-...
متن کاملCytotoxic Glucose Degradation Products in Fluids for Peritoneal Dialysis
During the standard heat sterilization process of the lactate–buffered peritoneal dialysis solutions, glucose (an osmotic active substance) degrades to form compounds called glucose degradation products which are cytotoxic and affect the survival of the peritoneal membrane. This case presentation is based on an observation of 224 aseptic peritonitis cases of unknown etiology. For the purpose of...

Background: Peritoneal dialysis is one of the most commonly used treatment methods for the patients with end stage renal failure. In recent years, the mortality rate of patients under this treatment has decreased; however, long-term survival is still an important challenge for health systems. The present study aimed to predict the survival of continuous ambulatory peritoneal dialysis patients. ...
متن کاملStrategies for improving long-term survival in peritoneal dialysis patients.
The incidence and prevalence of ESRD in the United States continues to increase. Currently there are over 26,000 patients maintained on peritoneal dialysis. Mortality rates have fallen over the past several years, but long-term survival remains poor, with only 11% of peritoneal dialysis patients surviving past 10 years. Cardiovascular disease accounts for most deaths, and dialysis patients have...
